I don't think 10,000,000 is really an exaggeration. That's only one surgeon for 3,000 people and with the low population densities in the 1860's if you needed an operation it could take days to get to get to one. Of course in those days surgery wasn't so common because surgeons didn't wash their hands or instruments so most patients died after major surgery. However everybody needs minor surgery from time to time so there were doctors in every town as well as dentists, midwives, et al who performed minor surgery.
10,000,000 might actually be an understatement but as Origyptian pointed out this is highly irrelevant.
The point is that you are taking the status quo to be inviolable. We would still be living in caves if everyone felt this way and there would be no pyramids to awe us.
The evidence is very clear that stones were pulled straight up the sides of five step pyramids one step at a time. This conclusion is inescapable and it still can not be reconciled by a paradigm that excludes it as a possibility and is founded in a chronology that can't be proven and is little better supported than all their other assumptions.
You can continue on ignoring the facts but soon enough there will be much more to ignore.

Man fears the pyramid, time fears man.
