Of course, an alternative is if the cutting technology was so fast that even prompt recognition of the mistake was already too late. This could happen with exceptionally productive cutting tool which could take some time to shut down. Just think of how long it takes to stop an industrial wood chipper if something mistakenly enters the maw! How much more "harm" can occur once the mistake is first noticed?
